Which Waiver Covers This Service

Supervised Living Residential is Virginia DD Waiver service 01-011, billed under Medicaid specialty 825. It is authorized under one waiver only.

Supervised Living Residential is available for eligible individuals through Virginia Developmental Disabilities (DD) Waivers, including the Family & Individual Supports (FIS) Waiver. Services are provided based on each participant’s approved Individual Support Plan (ISP).

CL Waiver

Supervised Living Residential provides a structured, home-like environment where individuals receive personalized support with daily living, community participation, and independent living skills. Services are provided based on each individual’s approved Individual Support Plan (ISP).

We proudly accept the Virginia Family & Individual Supports (FIS) Waiver. This waiver helps eligible individuals receive residential and community-based supports that promote independence, personal growth, and community inclusion.

Our program helps residents develop practical life skills, build confidence, and achieve greater independence in a safe, supportive residential environment.

Serenity Wellness is also enrolled with Virginia Medicaid under specialty 826 (Therapeutic Consultation — professional), 827 (Therapeutic Consultation — non-professional), and 831 (Non-emergency waiver transportation) — available across the CL, FIS, and BI waivers as authorized on the ISP.

Dr. Chiamaka Nnah, DNP, PMHNP-BC

I have spent more than a decade in psychiatric nursing, and as a first-generation Nigerian provider I know what it means to move between worlds. Again and again, I have watched adults with developmental disabilities be asked to fit into systems that were never designed with them in mind. I founded this program because I wanted to make a way for them.

This home is built as a culturally competent environment where mental health support is woven into everyday life — a team trained in behavioral health, with psychiatric consultation available through our practice — so that every resident has the aid they need to truly navigate supervised living, not simply be housed in it.

We see each resident as a human being first: valued, capable, and important to society. Our promise is to walk beside every person toward greater independence, and to make that transition a smooth one — at their pace, and on their terms.

Admissions

We'll Tell You Plainly Whether We're The Right Fit

We serve adults whose support needs can be safely met in a four-bed community setting with the staffing pattern above. No one is placed to fill a bed.

We cannot currently admit

Individuals with active aggression, self-injurious behavior within 15 days of the referral, active safety concerns, or a documented history of inappropriate sexual behavior. The home is not wheelchair accessible.
